Frequent Questions

Q. My form only has seven digits on the top for the form number?
 

 

A. It originally was to have nine, the printer made a mistake and it was too late to correct, over 200,000 forms will have the seven digits instead of nine, if you are using a 7 digit form and it has to be resubmitted due to an error start the rejected form number and mark the last two bubbles zero.

   

Q. I actually stop a car in another state, what is the Patrol area? Are there Patrol Area's in other states?
 

 

A. No there aren't, use the county where the stop was initiated. Not where the vehicle came to rest.
 

Q. Do I staple the Stop Form to the citation?
 

 

A. No, never staple to anything, any tears, folds, punctures may cause a rejection during the scanning process. If a rejection would occur the form will be sent back to your agency and you would have to resubmit. Shipping labels were sent with the forms to your agency, the forms are to be mailed to

All of Januarys before 15th of February, all of February's before March 15 and so on. The project currently is set to terminate at the end of this year.
 

Q. What is our Patrol Area?
 

  A. If you are a city less than 5,000 population, a Sheriff's Department in a county less than 20,000 or a State Enforcement Agency your patrol area will be 001. If your not, you will need to refer to the correspondences from the Division of Criminal Justice Services on directions in formatting Patrol Area's.
 
Q. How do I assign Officer Identifiers?
 
  A. Any way you wish, it must be four (4) digits. The Chief Executive Officer should assign the numbers. No one other than the CEO needs to know who the officer is.
 
Q. I don't want to ask someone what their race is.
 
  A. You should answer this by what you perceive their race is. Much of the information is based on the officers visual perceptions.
 
Q. How do we send the forms back to your agency?
 
  A. US Mail preferred. Put only the forms filled out during that calendar month. Any voids (torn, mistakes made, etc) together, Resubmissions together, and original submissions together, but separated from one another.
 
Q. I can't open the CD sent to my agency.
 
  A. Call 304-558-6080 Bob Tipton, we will try to help with any issues you might have.
 
Q. I still have forms from 2005, the full page forms. What should I do with them.
  A. You can discard the old forms from the 1st attempt at this project. They are not compatible and there is no need to retain them.
