- If the youth is living at home, the
parent or legal guardian must provide proof of residency documentation
- Tax records indicating a WV physical
address
- Current utility bills indicating WV
physical address (not a P.O. Box number)
- Residential rental and lease agreements
for a WV residence
- Mortgage documents for a WV residence

- A W-2 form not more than 18 months old
with a WV physical address
- A current concealed weapons permit with a
WV physical address
- A motor vehicle registration card in the
name of the applicant issued by the Division with a WV physical address
- WV Voter's Registration Card
- Proof of receipt of WV Public Assistance
- WV Homestead tax exemption
