PROCEDURES FOR DRIVING RECORD REQUESTS
YOUR RECORD WILL INCLUDE THE FOLLOWING INFORMATION: DRIVER’S LICENSE NUMBER, NAME, ADDRESS, DATE OF BIRTH, LICENSE ORIGINAL ISSUE DATE OR TRANSFER ISSUE DATE IF APPLICABLE, ALL CONVICTIONS AND ALL WITHDRAWALS (REVOCATIONS/SUSPENSIONS) WITHIN THE LAST FIVE YEAR PERIOD.
INDIVIDUAL REQUESTS:
You may request of copy of your own driving record in person
at any branch office of the DMV or by mail by providing the
following:
- Completed driving record request form (DMV-101-PS-1)
- A copy of your state issued photo identification
- A $5.00 processing fee if you provide your name and driver’s license number or
- A $6.00 processing fee if you do not have your driver’s license number (Must include DOB and/or SS Number)
Please clearly indicate where the driving record is to be mailed. (If Applicable)
ALL OTHER DRIVING RECORD REQUESTS:
- Completed driving record request form (DMV-101-PS-1)
- Completed Release Authorization form (DMV-101-PS-2)
- A copy of the requestor’s state issued photo identification (ex: spouse, parent, friend, etc.)
- A $5.00 processing fee per request if you have the driver’s license number and name or
- A $6.00 processing fee per request, if you do not have the driver’s license number. (Must include DOB and/or SS Number)
DMV REGIONAL OFFICES ARE PROHIBITED FROM DISPENSING DRIVING RECORDS TO ANYONE REQUESTING ANOTHER INDIVIDUAL’S RECORD.
All requests for a driving record for an employee must have the completed DMV-101-PS-1 and DMV-101-PS-2 unless the requests are made on company or business letterhead and provides a legitimate reason along with requestor’s photo identification.
Attorneys may provide their State Bar Number in lieu of a copy of the state issued photo identification. If the requested record is for their client, the DMV-101-PS-2 is required. If the requested record is not for their client, the Attorney must provide a letter detailing the need for the record as defined by the Uniform Motor Vehicles Records Disclosure Act, on letterhead and including a case number if possible or a subpoena duces tecum.
