FOR QUALITY CONTROL OF COMMERCIAL SOURCES.  NOTE 3.

The following sources are approved for limited application use only.  See qualifying statement on the test 
report to determine which applications are not suitable for this material.

Aggregate from the above named companies and producing sites have been sampled and tested in compliance 
with the 1980 Construction Manual.  Said tests have been evaluated with respect to the Standard Specifications
2000 and the sources are identified as supplying materials which have been found to meet the requirements of 
said specs, exceptions noted above.  Additional sources and/or types of material will be sampled and tested as
outlined above and corresponding evaluations will be supplied as an addendum to this report.  If District and/or 
Contractor personnel want additional sources evaluated, a request for pretest service should be made to the 
Materials Control, Soils and Testing Division (MCS&T Division).  When the type and source of material which
has current approval is used on a State job, District personnel should request coverage for same in the usual 
manner but a complete description of material source and quality check lab number must be provided.
Note 1 :   Sources sampled and tested this quarter and assigned new 
           Approval Numbers.
Note 2 :   Fine aggregate/Silica Sand from and/or about the James River 
	   has demonstrated under both field and lab conditions a marked 
	   tendency to increase the mix water demands of portland cement 
           concrete, and to cause more than the normal amount of air.  
           This effect is associated with the shape of the fine aggregate 
           particles. Therefore, considerable caution should be exercised 
           when this material is used in portland cement concrete.
Note 3 :   Because of the additional qualifications required for Item 402, hot laid bituminous concrete
	   skid resistant pavement, this list of sources and the corresponding report numbers may not
	      be used for approval of any quantities of said item unless otherwise noted.  Notification of
	      acceptable and potential skid resistant aggregate sources and means of evaluation are 
	      contained in the "List of Potential Skid Resistant Sources and Ratings" issued 
	      March 11, 2002
