Item Sheets Regulations |
- Individual letter size (8˝”
x 11” or 213mm x 293mm) sheets printed at 80 characters per line,
60 lines per page in non-proportional portrait mode using standard
MS-DOS (OS/2) functions.
- The schedule of item sheets
shall be printed on paper of a quality equal to that contained in
the proposal form issued by the WVDOH.
- Each page of the printed
schedule of items shall have a “check” value (automatically
generated by the software) printed at the bottom. Each schedule of
items page per bid shall have the same check value, with no pages
missing. This check value is used by the WVDOH, as part of the
analysis process, to verify that the electronic bid (diskette)
contains the same data as the paper bid. In the event of a
discrepancy between the printed bid and the electronic bid, the unit
bid prices on the printed bid shall govern.
- In addition to the
signatures required in the issued proposal, the bidder or a
qualified and authorized agent must also sign the EBS signature
sheet generated with and considered a part of the schedule of items
sheets.
- The bidder shall substitute
the executed EBS schedule of items sheets and accompanying signature
page for the schedule of items sheets which were originally included
in the issued proposal.
Bid Diskette
If the bidder elects to incorporate EBS schedule of item sheets in the
bid proposal, the bidder must also include a copy of the EBS bid in
electronic diskette (3 1/2” 1.44 MB) form in the bid envelope when
submitting the bid proposal to the WVDOH.
Each bidder must submit a
separate diskette for each bid proposal submitted. All diskettes become
the property of the WVDOH.
The bidder must label each
diskette. The data on the label must include the bidder’s company
name, FEI number, letting date, and call number of the applicable
electronic bid.
