Download the order form to request proposals, plans and cross-sections and send it to the WVDOT Division of Highways.

Invoicing will be through the WVDOT Division Of Highways, Finance Division, the following month.

You will need the call number and project number of the items you are requesting in order to fill out the document. To find this information, view the Notice \w Bid Items & Quantities section of the specific letting for which you are requesting information.
